먼저, Antibiotics는 the most common cause of anaphylaxis in nursing management이고, Latex는 바나나, 키위, 아보카도등과 같은 cross-sensitivieis를 가지고있다는걸 기억하면서 정리를 시작해보기로!

1. Anaphylactic Reaction

• repeat exposure to allergens

• IgE antibody-mediated response

• activate mast cells, basophils and eosinophils

• release inflammatory mediators; cytokine, histamin etc

• severe inflammation, edema, airway contraction, urticaria and vasodilation to life threatening

Symptoms: severe hypotension & tacgycardia

• arrhythmias, and cardiac arrest

• bronchospasm (severe wheezing)

• angioedema (lips, tongue, facial swelling)

• ariway obstruction

• itching, urticaria, flushing, redness, stomach ach, acid reflux, vomiting

Intervention:

• remove allergens immediately, call for help and get code cart

• give peinephrine FIRST to prevent IgE medicated response (IM, IV) - epipen is preferred: mid to outer thigh

• can repeat every 5-15 mins until symptoms improve

2. Angioedema (emergency)

Symptoms:

• urticaria, lips, eyes, tongue, and facial swelling

• compromised airway due to airway edema

• can quickly become respiratory emergency

Treatment:

• (treat as abaphylaxis)

• responds to epi, antihistamines, steroids

• prepare for difficult intubation/cricothyrotomy on standby

3. Latex Allergy

• Type 1 Hypersensitivity (IgE); immediate, acute allergy response to natural rubber latex

• Type 4 hypertensivity (cell-mediated); contact dermatitis characterized by itching, swelling, redness, blistering, small raised bumps, and the formation of crusts and thickening of the skin (6-28 hours)

Products with cautions: gloves, condoms, balloon-tipped catheters, dental dams, balloons, adhestives, vial stoppers, paciders, adhesive or elastic bandages, blood pressure cuff etc

Latex cross-sensitivities: 30-50% with Banana, kiwi, avocado, chestnut, papaya, white potato, tomato

4. Allergy Skin Test

• ensure that informed consent was obtained

• prepare the client's test site with soap and water

• use alcohol to remove any oil from the skin

• ensure availability of equipment for treating anaphylaxis (ACLS cart, epi, antihistamine, steroid)

• instruct the client to avoid taking corticosteriods and antihistamines for 5 days before testing

• ensure that the client remains in the waiting room or office for a minimum of 30 minutes after the injections to monitor for any adverse effects

내가 헷갈렸던 포인트/한번 더 정리하고싶은 개념

1) Anaphylaxis와 angioedema

Anaphylaxis는 전신적인 hypersensitivity reaction으로, 갑작스러운 hypotension, bronchoconstriction, wheezing, stridor가 동반될 수

있어 가장 우선적으로 epinephrine IM 투여가 필요하다. 반면 angioedema는 lips, tongue, face swelling이 특징적이며 airway obstruction 위험이 가장 중요했다. 특히 ACE inhibitor와 관련된 angioedema는 allergic mechanism이 아닐 수도 있다는 점이 헷갈렸지만, 결국

airway assessment가 priority라는 점이 핵심이었다

- 둘 다 swelling이 있어서 헷갈렸는데, angioedema는 localized swelling이 중심이고 airway obstruction 여부를 가장 먼저 봐야 한다는 점에서 차이가 있었다

2) Latex allergy에서는 banana, avocado, kiwi 같은 cross-reactive food를 함께 연결해서 기억하기

3) 또한 allergy skin test는 검사 전 antihistamine 중단 여부와 검사 후 anaphylaxis 발생 가능성까지 함께 고려하기!

문제/케이스 + 내 풀이 과정

이 문제에서 내가 헷갈렸던 이유는 강의에서는 antihistamine이랑 corticosteroid를 검사 5일 전에 중단한다고 배웠는데, 문제에서는 3일 전이라고 나와서 “어? 숫자가 다르네?” 하고 바로 틀린 선택지라고 생각했기 때문이다..

그래서 처음엔 a를 지우고 d만 정답이라고 판단했다. 근데 다시 보니까 이 문제의 핵심은 정확히 5일 자체가 아니라, allergy skin test 전에 antihistamine이나 steroid를 중단해서 allergic reaction이 제대로 나타나게 해야 한다는 원칙이었다. 즉, 검사 결과 정확성을 위해 면역 반응을 억제하는 약물을 미리 중단해야 한다는 개념이 더 중요했던 거였다. 나는 숫자 자체에 꽂혀서 판단했는데, NCLEX 스타일 문제는 단순 암기보다 왜 그렇게 하는지를 이해하는 게 더 중요하다는 걸 다시 느꼈다. 그래서 이후에는 며칠인지 외우는 것보다 “검사 결과 왜곡 방지”라는 흐름으로 연결해서 공부하려고 했다. 처음에는 숫자 하나에만 집중해서 오답이라고 판단했는데, NCLEX 스타일 문제는 세부 숫자 암기보다 “왜 그렇게 하는지”를 이해하는 것이 더 중요하다는 걸 느꼈다. 그래서 이후에는 단순 기간 암기보다 검사 결과의 정확성을 위해 면역 반응을 억제하는 약물을 중단한다는걸 기억하는게 더 중요하다!
